Centro Medical and Surgical Centre recognizes and respects your privacy when you use the Patient Portal service to securely access your electronic medical records on-line.
As your Personal Information includes your personal health information, we need your express consent to collect your personal health information for the
purposes of facilitating the services. By providing your personal information to us, you agree that Centro Medical Clinic may use your information only
for the purposes for which it is collected or for a use consistent with that purpose. Any personal information collected by Centro Medical Clinic is
collected in compliance with the Freedom of Information and Protection of Privacy Act, RSBC 1996, c.165. Centro Medical Clinic does not disclose any
personally identifiable user information to other organizations or individuals, except as required by law, such as reporting infectious diseases pursuant
to statutory obligation, reporting abuse to regulatory bodies, to law enforcement in certain circumstances, to comply with a subpoena or court order or
records, or for other statutory or legal purposes.
We only collect and use your personal contact information for the following purposes: to confirm your identity when you register for the Patient Portal;
to deliver the services that you have requested; to manage your account and provide you with customer support. Your e-mail address will only be used to send
important clinic related service messages or changes to the services. You may withdraw your consent to these uses, but if you do, we may not be able to continue
providing the services as requested
You are responsible for ensuring that you do not disclose your account access information to third parties, this includes but is not limited to not disclosing
this information to anyone, ensuring that sufficient security software is installed and running on your computer, smartphone or tablet, that your network firewall
is of sufficient quality and that your wireless connection is encrypted to a degree that will prevent third party access. Do not share your User ID and password
with anyone. If you suspect that your password has been compromised for any reason, you should change it immediately.
Centro Medical and Surgical Centre and our technology partners use safeguards to protect your personal information against loss or theft and unauthorized access; including when
transferred between your computer and our server. Your access to the Patient Portal is a secure connection. For additional protection, after 3 unsuccessful login
attempts a user will be locked out and the website will automatically log out after 15 minutes of inactivity. Only our authorized employees, who are bound by a
confidentiality agreement, can access your personal information when required to perform their duties.
Centro Medical and Surgical Centre reserves the right to change its privacy policy. Any revisions will be posted to the website accordingly. If you have any questions or concerns
